QA/Documentation Specialist

The Opportunity: The QA/Documentation Specialist is responsible for ensuring the accuracy, completeness, and regulatory compliance of all clinical documentation across the organization. This non-clinical role focuses on supporting chart reviews, obtaining care plans, and gathering documentation needed to establish medical necessity, all in alignment with Medicare/Medicaid requirements, state program rules, and accrediting body standards. Working under the direction of our Ohio-licensed clinical team, the specialist helps maintain high-quality documentation integrity without performing direct patient care or independent clinical decision-making. This position is remote and offers you the flexibility of working from home. Key Responsibilities • Regulatory Compliance & Documentation Review • Ensure all documentation meets Medicare and Medicaid Conditions of Participation (COPs), accrediting body recommendations, and state program rules and regulations related to care delivery. • Conduct thorough chart reviews to ensure accuracy, completeness, and compliance at all touchpoints of care. • Verify that all required documentation elements are present, timely, and consistent with internal policies and regulatory standards. • Understand the impact of documentation quality on overall care delivery, branch compliance, and financial integrity. • Quality Assurance Support • Support the QA team by identifying documentation gaps, trends, and areas for improvement. • Assist in obtaining care plans, physician orders, and supporting documents necessary to establish medical necessity. • Follow structured guidelines and established processes under the direction of Ohio-licensed clinicians. • Team Leadership & Professional Standards • Serve as a professional role model for QA Nurse Team Members and other clinical staff by demonstrating high standards of accuracy, cooperation, conduct, and professionalism. • Promote a culture of compliance, quality, and continuous improvement within the QA team. • Confidentiality & Information Management • Maintain strict confidentiality and ensure all patient information is handled in accordance with HIPAA and internal safeguards. • Ensure secure handling, storage, and sharing of documentation across teams. • Issue Resolution & Reporting • Investigate and escalate documentation-related issues, concerns about clinical performance, or deviations from standards to the Director of Clinical Services. • Collaborate with internal teams to resolve documentation discrepancies and ensure timely corrections. Qualifications Required • Bachelor’s degree preferred (Healthcare Administration, Public Health, or related field) or equivalent experience. • Experience in healthcare documentation, quality assurance, medical records, or compliance. • Strong understanding of healthcare regulatory environments (Medicare, Medicaid, state programs) preferred. • Excellent attention to detail and ability to review large volumes of documentation with accuracy. • Strong communication, organization, and problem-solving skills. • Ability to work independently while following structured guidelines and direction from licensed clinical leaders. Preferred • Experience working in home care, hospice, or managed care environments. • Familiarity with EMR systems, care plans, and clinical documentation workflows. • Knowledge of accrediting body standards (e.g., CHAP, Joint Commission).
