We partner with payers, employers, and providers to expand access to quality healthcare across the system. We extend that same personalized model to support women through the midlife transition. We provide expert guidance for symptom management, long-term health planning, and navigating the physical and emotional changes of menopause. Role Description Your North Star: Redefine the midlife experience for women by providing expert mental health guidance that prioritizes education and early intervention. Serve as a clinical partner to our patients, helping them manage the emotional, cognitive, and physical challenges of menopause transitions through compassionate, evidence-based therapy and personalized care planning. Your key responsibilities will include: • Provide one-on-one virtual care visits for Pomelo patients • Establish individualized care plans to meet specific health needs based on evidence-based standards • Complete behavioral health screenings, ensuring appropriate responses including diagnostic, therapy, and behavioral health interventions to midlife patients • Perform ongoing psychosocial assessment of patients, providing reassurance and building rapport • Identify and address barriers that have been identified by patients • Reduce care gaps (missed appointments, medication management, etc.) by frequent engagements with patient • Collaborate with all involved teams and support services to ensure that patient and family needs are met • Document all care/interactions in a timely fashion and escalating to appropriate multidisciplinary teams, as needed • Provide high quality care in accordance with industry and Pomelo standards • Ensure continuous compliance with all relevant licensing board requirements to maintain an active clinical license • Participate in continuous quality improvement efforts to improve our ability to provide the highest quality care to patients • Support group care, as needed Who you are • Possess an active LMHC or LICSW license in the state of Massachusetts and willingness to obtain licenses in all US states • 5+ years of experience in midlife mental health care, specifically addressing the intersection of hormonal shifts and psychiatric symptoms (depression, anxiety, and trauma).
