Compensation & Incentives • Competitive base hourly rate • Team members are eligible for additional compensation based on the timing of hours worked, as follows: • Evening Differential: An additional $1.50 per hour will be paid for all hours worked after 7:00 PM Eastern Time. • Late-Night Differential: An additional $2.00 per hour will be paid for all hours worked after 10:00 PM Eastern Time. • Weekend Differential: An additional $1.50 per hour will be paid for all hours worked on Saturdays and Sundays, regardless of time of day. • Shift differential pay is calculated in addition to base hourly wages and will be reflected on the corresponding payroll cycle. • Bonus potential: Earn up to $1,500 per month based on patient engagement and performance goals Key Responsibilities • Conduct thorough assessment calls with patients to gather relevant information about their health status, concerns, and needs • Identify and escalate patient needs, including refill requests, appointment scheduling, and connecting patients with community resources • Develop comprehensive care plans based on patient assessments • Perform Behavioral Health Integration (BHI) assessments to evaluate and address the mental health aspects of patient care • Provide patients with relevant and clear information about their health conditions, treatment plans, and self-management strategies • Collaborate with patients to establish realistic and achievable health goals, aligning with their overall care plans • Assist patients in troubleshooting issues with medical devices and guide on setting up devices as needed for their care • Respond promptly to patient inquiries and calls, addressing their questions or concerns in a timely and efficient manner • Conduct follow-up calls • Regularly check and return voicemail messages from patients • Manage the rescheduling of appointments when necessary • Help foster a collaborative and positive work environment.
